Formula Health Limited (“Formula Health”, “Our”, “We” or “Us”)

All our usual terms and conditions and policies apply to Gift Voucher (also referred to as Gift Packages.).

1. Rules

1.1 The use of gift vouchers fall under the following terms and conditions: In order to purchase a Gift Voucher, you will need to provide a name, email address and contact phone number to complete the transaction through our Website.

Gift Certificates ordered through our Website will be delivered by electronic means to the email address you specify at the time of order. You may then forward this onto the intended recipient.

Our Gift Vouchers include unique reference numbers.

Our Gift Vouchers must be redeemed at the clinic as full or part payment of products or treatments or from items on our online shop.

A Gift Voucher cannot be used to purchase a further Gift Vouchers.

When ordering a Gift Voucher, you should ensure to enter your email address correctly. We cannot be held responsible if the address is entered incorrectly and someone other than the intended recipient uses the Gift Voucher.

Site offers and promotions or additional discounts do not apply when purchasing gift vouchers.

All Gift Vouchers have a 12-month validity*

*Vouchers will be extended if in-person appointments cannot be attended due to COVID restrictions.

Gift Vouchers cannot be exchanged for cash.

Gift Vouchers may only be redeemed against products purchased in the same currency as the Gift Voucher.

Where Products purchased online with a Gift Voucher are returned, monies owing will be refunded by a Gift Voucher.
