What is a Health and Wellbeing assessment? 

Health and wellbeing assessments are structured processes used to evaluate an individual’s overall physical, mental, and emotional state. They typically involve gathering information about lifestyle habits, such as diet, exercise, sleep patterns, stress levels and medical history. These assessments may include questionnaires, one-to-one discussions, physical measurements (such as weight and blood pressure), and relevant screening tools and tests (such as blood or stool test) to identify risks related to physical health or chronic conditions.

Clinicians use Health and Wellbeing assessments for many reasons. These include:
