Health and Wellbeing Assessments

Your Care Pathway with us:

Detailed discussion of your concerns
The GP will ask about:

  • Your current symptoms (mental and physical)
  • How long they’ve been going on
  • Triggers or stressors
  • Sleep, appetite, energy, concentration
  • Impact on daily life (work, relationships, motivation)

Medical and lifestyle review
They’ll usually take a broader view than just symptoms, including:

  • Medical history and medications
  • Lifestyle factors (sleep, alcohol, diet, exercise)
  • Possible physical causes (e.g. thyroid issues, deficiencies)

Possible questionnaires or screening tools
You might be asked to complete short assessments to help measure symptom severity, especially for anxiety or depression.

Discussion of next steps
This is a key part of a GP visit. Options may include:

  • Lifestyle advice or self-help strategies
  • Prescribing medication (if appropriate)
  • Referrals to a private psychiatrist or therapist
  • Blood tests or physical health checks
  • Ongoing follow-up appointments

What is a Health and Wellbeing assessment?

Health and wellbeing assessments are structured processes used to evaluate an individual’s overall physical, mental, and emotional state. They typically involve gathering information about lifestyle habits, such as diet, exercise, sleep patterns, stress levels, and sometimes medical history. These assessments may include questionnaires, one-to-one discussions, physical measurements (like weight, blood pressure, or fitness levels), and sometimes screening tools to identify risks related to mental health or chronic conditions.

What can I expect?

Clinicians use Health and Wellbeing assessments for many reasons. These include:

  • Increases awareness of personal health and lifestyle habits
  • Identifies potential health risks early
  • Supports early intervention and prevention of illness
  • Encourages healthier lifestyle choices (e.g. diet, exercise, sleep)
  • Helps set realistic and personalised health goals
  • Improves mental health by recognising stress or emotional concerns
  • Provides a clear baseline to track progress over time
  • Boosts motivation to make positive changes
  • Enhances overall quality of life and wellbeing
  • In workplace settings, can reduce absenteeism and improve productivity

Private GP consultations are typically used to help diagnose:

  • Common physical illnesses
Including infections, respiratory problems, digestive issues, skin conditions, and general unexplained symptoms
  • Chronic conditions
Like asthma, diabetes, high blood pressure, or long-term pain conditions—either diagnosing them or monitoring ongoing management.
  • Unexplained or overlapping symptoms
When symptoms could have multiple causes, GPs help narrow down possibilities and rule out serious conditions.
  • Need for further investigation or referral  A big part of a GP’s role is deciding if you need blood tests, scans, or referral to specialists (such as psychiatry, cardiology, or dermatology)
  • Holistic health factors
They also assess how lifestyle, stress, sleep, and physical health may be contributing to symptoms.
