We understand that occasionally our clients need to rearrange, adjust, or cancel their appointments. We require 48 hours’ notice for any appointment changes or cancellations as it is difficult for us to offer the practitioners / instructors time to another client at such short notice and this prevents us from helping those who are in need of an available appointment.
Any cancellations or changes with less than 48 hours notice will be charged in full for the cost of the appointment booked.
If you have cancelled or rescheduled an appointment more than once, we require all further appointments to be paid at the time of booking.
We take privacy very seriously at Formula Health and patient records are stored securely in our medical grade practice management software which is compliant with UK Data Protection Legislation ( GDPR Act 2018) and EEA Data Protection Legislation (GDPR-EU 2016/679).

The wearing of masks in enclosed spaces is encouraged but now remains at your discretion. We will continue to operate high hygiene standards throughout the clinic and our practitioners and instructors will continue to use PPE as directed by their governing bodies.
If you believe you may have had recent exposure to COVID 19 or are experiencing any symptoms, a negative lateral flow test prior to your appointment is sufficient in order for you to attend.

If however you have a concern or complaint about the standard of service you have received from an Osteopath or any member of staff within the clinic, please let us know immediately and follow the steps described on this page.
We have 3 free parking bays for our clients use, numbered 3, 4 & 41 – these are conveniently located opposite the front door to Formula Health and right next door to the clinic/garden. On arrival in the clinic you MUST enter your vehicle registration on the tablet in the reception area. The car park is operated by the landowner’s CCTV surveillance and will therefore issue fines to unauthorized vehicles that enter the premises.
If you do happen to receive a fine in error, despite having entered the correct details, please contact Formula Health and send an email of the parking fine (displaying the PCN, date and registration) and we will deal with Smart Parking’s mistake on your behalf.
